مقاومت خط‌مشی در طرح تحول نظام سلامت

چکیده

در سپهر سیاسی و خط‌مشی‌گذاری عمومی ‌می‌توان موارد متعددی از خط‌مشی‌های عمومی‌ را یافت که حتی با وجود ناکارآمدی، در برابر خاتمه‌یابی از خود مقاومت نشان داده، همچنان به بقای خود ادامه می‌دهند و علاوه بر هدر دادن منابع عمومی، ممکن است منجر به تشدید مسئله شوند. با توجه به اهمیت این موضوع، تاکنون در مطالعات حوزه خط‌مشی‌گذاری کشورمان مطالعات منسجمی‌در خصوص پدیده مقاومت خط‌مشی‌های عمومی‌ صورت نگرفته است.  بنابراین، هدف این پژوهش تبیین مفهوم «مقاومت خط‌مشی» و تعیین خصوصیات و ویژگی‌ها‌ی آن است. با توجه به غنای کم نظریه‌های مقاومت خط‌مشی، در این تحقیق رویکردی کیفی اتخاذ شد. همچنین برای گردآوری داده‌ها ضمن استفاده از راهبرد نمونه‌گیری نظری با 11 نفر از خبرگان سازمان‌های دولتی و نهاد‌های پژوهشی حوزه‌ خط‌مشی‌گذاری مصاحبه‌های نیمه‌ساختاریافته انجام شد. جهت تحلیل داده‌های گردآوری شده نیز از روش تحلیل مضمون استفاده شد. برابر با شبکه‌ مضامین ترسیم شده مشخص شد که مقاومت خط‌مشی‌های عمومی‌دارای 3 مضمون فراگیر، 11 مضمون سازمان‌دهنده و 41 مضمون پایه است. در پایان نیز جهت آزمون شبکه مضامین استنتاج شده و مطابقت آن با واقعیت‌های عینی، راهبرد مطالعه موردی به‌کار رفت و این شبکه با خط‌مشی طرح تحول نظام سلامت (به‌عنوان یکی از خط‌مشی‌های مقاوم در برابر خاتمه‌بخشی) تطبیق داده شد. نتایج این بررسی نشان می‌دهد که خط‌مشی طرح تحول سلامت به‌واسطه ویژگی‌هایی همچون: دوام نهادی، ابهام در جانشین‌پذیری، پیچیدگی بالا، هزینه‌های بالای راه‌اندازی، مشاهده‌پذیری کم و دامنه‌ فراگیر و همچنین در اثر پیشایندهایی چون درهم‌تنیدگی سیاسی-اجتماعی و تعارض منافع در برابر خاتمه‌بخشی، مقاومت کرده است.

In the political sphere and public policy-making can be found numerous instances of public policy that, despite inefficiency , resist on termination, continue to survive and in addition to wasting public resources and disrupting bureaucratic structure, may Lead to aggravation of the problem. Given the importance of this issue, unfortunately, so far in the field of policy-making in our country, no coherent studies have been conducted on the phenomenon of public policy resistance. Therefore, due to the existing ambiguity, the purpose of this study is to explain the concept of "policy resistance" and determine its characteristics and features. Due to the richness of policy resistance theories, a qualitative research was used in this research. Also, semi-structured interviews were conducted with 11 experts from government organizations and institutions in the field of policy to collect data. Thematic analysis method was used to analyze the collected data. According to the Thematic Network, it was found that the resistance of public policies has 3 Global Themes , 11 organizing themes and 41 basic themes. Finally, using case study strategy to test the Thematic Network with objective facts. this network was adapted to the health system reform plan (as one of the policies resistant to termination).
